
Virtual ordering options for direct to patient care
Myriad is dedicated to finding solutions for healthcare providers. To better serve you and your practice, we offer two different processes to ship our collection kits directly to your patient.

Via our online portal, MyriadPro
- Login to MyriadPro, our secure online portal. To register for MyriadPro – click here. For instructions on how to register – click here.
- Fill out an electronic test request form (eTRF) for Myriad myRisk®, and be sure to indicate that you want the test kit delivered to the patient for sample collection by checking the box.
- Once you ‘submit’ the test request form, the Myriad team will send this completed TRF along with a test kit directly to the patient’s home. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
- The patient will then need to complete the specimen collection and mail it back to Myriad with the signed TRF AND a front and back copy of their insurance card inside the kit for their sample to be processed.
- Once Myriad has received the patient’s sample – turnaround time, billing, and report delivery will proceed as normal.
- For those that are new MyriadPro users – your results will also be delivered through MyriadPro and you will receive an email when results are posted.
Via email/fax
- Fill out a myRisk test request form (TRF) as you normally would – download TRF here.
- Use this coversheet to indicate that you want the test kit sent directly to the patient’s home.
- Email or fax (801-584-3615) the TRF with coversheet to Myriad.
- Myriad will review and then mail your completed TRF along with a test kit directly to the patient. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
- The patient will then need to complete the specimen collection and mail it back to Myriad for their sample to be processed.
- Once Myriad has received the patient sample – turnaround time, billing, and report delivery will proceed as normal.
*IMPORTANT INSURANCE INFORMATION: Myriad will need a front and back copy of your patients’ health insurance information. There are two ways this can be provided: 1) Provider sends copy with email/fax 2) Patient provides copy in test kit when mailing back to Myriad
